Community Outreach & Network Quality (CONQ) Team

The CONQ Team works seven days a week to engage patient families with open care gaps, connect families to their medical home, and address social drivers of health that may prevent access to care.

Using MyChart and phone outreach, the team supports practices in closing care gaps and improving preventive care outcomes across the network.

Request a Scorecard Review or Office Visit

Providers may request virtual or in-person meetings to review quality scorecards, patient lists, and improvement strategies.

During each visit, our team can:

• Review your office-specific quality performance
• Identify high-priority care gaps
• Share evidence-based best practices for closing care gaps and referral services

Care Everywhere

Care Everywhere enables secure health information exchange between RCHN and other Epic-connected organizations, improving continuity of care and reducing duplicate testing.
