Help for Tough Health Problems

We help patients and their families manage their medical care.

Complex Case Management is a free service that gives extra help to people who need a lot of medical care. It’s meant to support you as you work on getting healthier.

Who Can Get Complex Case Management?

You might get this help if your child has:

  • A health problem that needs many doctors or treatments
  • A long-term illness
  • Lots of visits to the emergency room
  • Many hospital stays

These extra services are part of your current health plan. The following plans can get this help:

  • Sharp Health Plan HMO RCHN
  • Health Net HMO RCHN
  • United Healthcare HMO RCHN

Complex Case Management Services

Here’s how we can help your family:

  1. Care Planning
    We’ll work with you to make a care plan so your child gets the care they need.
  2. Better Care Coordination
    We can help you make appointments and plan for your child’s care when they turn 19 and move to adult care.
  3. Health Support
    We’ll encourage healthy habits and help you learn more about your child’s condition and treatment. We’ll also show you how to find helpful resources.
  4. Community and Support Services
    We can connect you with services in your community, like help with food, housing, or disability support.

Who Provides Complex Case Management Services?

You’ll have a special nurse called a Nurse Care Navigator who works with you to make sure your child gets the care they need. This nurse will also talk with your child’s doctors and other health care providers to give extra support.

They can also work with people you choose, like:

  • Doctors, specialists, and nurses
  • Therapists and social workers
  • Pharmacists and nutritionists
  • Caregivers and family members who help

How to Get Complex Case Management Services

There are three ways to get started:

  1. Your doctor, health plan, or another provider may contact you.
  2. You can call us at 858-309-7979 or 877-276-4543.
  3. You can call your health plan to ask questions and see if you qualify.
